  • Records not summarized: laboratory reports, inpatient progress notes, and anesthesia records.
  • Only the initial and final evaluations are summarized: Physical, Occupational, Acupuncture, Chiropractic Therapy evaluations.
  • For radiological studies all findings (normal and abnormal) are reviewed at the first occurance in the case, abnormal findings are then reviewed (x-rays, CT, MRI, etc.).
  • While reviewing outpatient follow up visits with the same provider, only the complaints and/or progress of the patient’s condition and physician’s recommendations and/or opinions are summarized.
